- Enclose the fee by Demand Draft, Cash or Online, in favour of Dean, UCSSH, MLSU, Udaipur (Raj.), and write the DD number, date and bank on the form.
- Sign the declaration in Part B, with the date and place.
- Attach the enclosures listed below and submit the form to the Department of Visual Arts.
