Required Documents for Verification
- A filled checklist downloaded by scanning the QR Code given in the notice, with all details completed.
- Printout of the OJAS online application hall ticket.
- Std. 10 (SSC) passing certificate.
- Std. 12 (HSC) mark sheets of all attempts and passing certificate.
- School Leaving Certificate (2 copies).
- G.N.M. mark sheets of all years and all attempts.
- B.Sc. (Nursing) mark sheets of all years and all attempts.
- Registered Nurse / Registered Midwife (G.N.C. Registration) certificate.
- Proof of Gujarat Nursing Council registration status - whether permanent or temporary; if temporary, valid-until date and all renewal receipts, or the permanent registration receipt.
- Computer Basic Knowledge certificate as per the General Administration Department circular dated 13-08-2008.
- For Scheduled Caste (SC) category candidates: caste certificate issued by the competent authority. Additionally, for caste verification, a separately downloaded and filled caste-verification form (as per the linked format) along with the required supporting documents must be submitted.
- For Socially and Educationally Backward Class (SEBC) candidates: the prescribed non-inclusion (non-creamy layer) certificate format, with the certificate number and date as mentioned in the online application, valid as on the date of advertisement.
- For Economically Weaker Section (EWS) candidates: the prescribed EWS certificate format.
- For candidates with physical disability: a Superintendent/Civil Surgeon medical certificate as prescribed under the General Administration Department circular no. Parcha/102008/469540/G-2 dated 01-12-2008.
- For Ex-Servicemen: original Discharge Book and certificate/identity card issued by the concerned department, along with original certificates of a minimum 5 years of military service including E.C.O./S.S.C.O. service, plus attested copies of each.
- For government employees: a letter confirming that the concerned department/office was informed within 7 days of applying online, plus a "No Objection Certificate" from the office, original and attested photocopies.
- If the candidate's name has been changed, supporting proof of the name change.
- All candidates must bring the following original certificates/documents, arranged in the order listed below, along with one self-attested photocopy of each:
Additional Verification Instructions
- Candidates must have completed Gujarat Nursing Council registration at the time of applying (after passing B.Sc. Nursing / G.N.M.); the certificate bearing the registration number mentioned in the application must be produced.
- A photo ID proof (Driving Licence, Voter ID, Aadhaar Card, or Passport) must be brought in original along with one attested copy.
- Married female candidates whose name differs between the application and certificates must bring the original and an attested copy of the Marriage Registration Certificate / Government Gazette notification.
- The photograph uploaded on the OJAS online application printout must match the photograph the candidate brings; the candidate's signature must also match the one on the application printout.
- Any discrepancy found between submitted documents and the online application details will be decided by the Recruitment Committee, and the decision will be final and binding on the candidate.
- Any discrepancy discovered in submitted documents in future will also be binding on the candidate as per the decision of the Government; candidates must furnish an undertaking to this effect.
- Candidates must download the document-verification checklist from the website and bring it along.
Important Insrtuctions
- Report to the venue strictly as per your serial number's assigned time slot (Sr. No. 1-80: morning slot; Sr. No. 81-149: afternoon slot).
- Arrange all original documents and the self-attested photocopy set exactly in the order given in the checklist above before arriving.
- If a candidate fails to produce the required documents during verification, the decision of the Staff Nurse Competitive Exam Committee on that matter will be final and binding on the candidate.
- After the scheduled verification days conclude, no further representation on any ground will be considered.
- Only the candidate will be permitted entry to the verification venue.
